Join us for our next Caring and Connections brunch on February 12th at 10AM. We will be highlighting information provided by the Compassus Hospice Group. The event will last for about one hour and a free brunch will be served. Everyone is invited. This is a community event and open to the public. There is no cost, and reservations are not needed to join us monthly to learn and ask questions. We look forward to seeing you for brunch.

Residents of The Lamoine receive a value-added service through Western Illinois Home Provider Care. Through this partnership, residents are given enhanced oversight and care from a Nurse Practitioner (NP) that may include: Residents of The Lamoine receive a value-added service through Western Illinois Home Provider Care. Through this partnership, residents are given enhanced oversight and care from a Nurse Practitioner (NP) that may include:

  • Comprehensive, tailored assessments of health needs.
  • Family support to promote evidenced-based education on disease processes and care techniques.
  • Care coordination between the facility, primary care providers, specialists, family members, and other ancillary services so that the resident’s healthcare needs are met. 
  • Promoting wellness and preventative care, including screenings, vaccination programs, and lifestyle counseling.
  • Personalized care plans that address overall care needs, ensuring residents receive the appropriate level of support based on personal preferences and health status to optimize overall independence.
  • Managing symptoms of acute or chronic illnesses to enrich the resident’s quality of life. 
  • Regularly-scheduled rounding and in-house convenient care to address ongoing or immediate needs — offering peace of mind to families previously needing to coordinate schedules and transportation to and from appointments.  

Meet Mandy

Mandy Kubis, MSN, APRN, FNP-BC, WCC serves as Nurse Practitioner for Western Illinois Home Provider Care. Her healthcare journey began in 2008 and has since accumulated a wealth of diverse medical experience including oncology, med-surg, and emergency room care. Mandy found her true calling for in-home care in 2015 when she became a Case Manager for Western Illinois Home Health Care. She became wound care certified (WCC) in 2019 and became a Family Nurse Practitioner (FNP) in 2023. 

In her spare time, she enjoys crafting, cheering on her children at their various sporting events, and traveling internationally with her husband.
